About the Personalised Care & Health Coaching Collaborative
We are a collaborative of health and care organisations, drawing together expertise in: - quality assured training provision, health coaching and acute, community, social care, primary, VCSE professional settings. Our purpose is to share high quality education, learning and development to support organisations to build a culture of personalised care, which starts with health coaching conversations and asking the open question ‘what matters to you?’
Our Courses
Introduction to Personalised Conversations (Part 1)
4 half days - 15 hours - virtual online cohorts - 12 people
This 2 day training (over 4 half days) is suitable for everyone working in health and care systems. Suitable for practitioners within the Health & Social Care Workforce & Community Teams (Clinicians, AHP’s, Health and Wellbeing Coaches, Care Co-ordinators, Social Prescribers & Social Workers. The course includes:
Module 1 - Intro to Personalised Care and the ‘PERSON’ Model
Module 2 - Intro to Personalised Care Coaching Communication Skills
Module 3 - Tailoring your conversations to the Person
Module 4 – Navigating Challenges in conversations
Personalised Care & Health Coaching Conversations Skills (Part 2)
4 half days – 15 hours - virtual online cohorts - 12 people
Part 2 builds on Part 1, and offering a further 2 days programme. Part 1 and part 2 together create a 4 day programme for Health and Wellbeing Coaches, Health and Care professionals with a special interest e.g. GP’s, Allied Health Professionals, Social Workers or Trainer Health and Care professionals. Please note part 1 must be completed to participate in part 2. The course includes:
Module 5 - Intro to Coaching Models and reflective practice
Module 6 - Personalised care Coaching Mindset 1: A personalised approach
Module 7 - Personalised Care Coaching Mindset 2: A solutions focus
Module 8 - Learning in Practice and Reflection with Peers
